Job summary

Mercy Home Health Care is seeking a Full-time RN WOCN Consultant based from our Philadelphia area branch to support the City of Philadelphia. The role involves providing expert wound care in patients' homes, coordinating treatment plans, and educating patients and families on wound management and prevention.

Required are an active PA RN license and WOCN certification, with strong interpersonal and problem-solving skills.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in the state of PA.
• Three years of experience as a professional care nurse.

Responsibilities

• Provide one-to-one wound care to patients in their homes and coordinate treatments.
• Educate patients, families, and staff on wound management and prevention.
• Work as part of Mercy Home Health Care team to deliver compassionate, patient-centered care.

Mercy Home Health Care has a need for a Full-time (1.0 FTE - 40 hours weekly) RN WOCN Consultant. This position is based from our South Philadelphia branch office and will support the City of Philadelphia.

The WOCN is responsible toprovide wound care to the patients of Mercy Home Health. Coordinates treatment to provide comprehensive wound care assessments and plan of care including goals, implementation of treatment, evaluation and outcomes for patients. Provides patient, family, and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections, and other therapies.

Current RN licensure with WOCN required or

Certified prepared wound, ostomy, and continence nurse (WOCN).

Valid driver's license and car insurance.

Strong interpersonal and communication skills.
Demonstrates ability to use problem solving, critical thinking and priority setting skills.

Minimum qualifications
