Mercy Home Health Care is seeking a Full-time RN WOCN Consultant to service Delaware and Chester Counties from our Newtown Square branch. You will provide wound care and coordinate comprehensive treatment plans in patients' homes.

Applicants must hold PA RN licensure with WOCN certification or be prepared to obtain it, plus a valid driver’s license. The role emphasizes patient-centered care and collaboration with a supportive team using advanced technology.

Qualifications

• Graduate of an approved nursing education program.
• Current PA RN licensure.
• Three years' experience as a professional nurse.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide one-to-one wound care to patients in their homes.
• Coordinate treatment plans and educate patients and families on wound management.
• Utilize evidence-based care to prevent pressure ulcers and manage wounds.
• Collaborate with care teams and use technology to document care and outcomes.

Mercy Home Health Care has a need fora Full-time (1.0 FTE - 40 hours weekly) RN WOCN Consultant. This position is based from our Newtown Square branch office and supports Delaware and Chester Counties.

The WOCN is responsible toprovide wound care to the patients of Mercy Home Health. Coordinates treatment to provide comprehensive wound care assessments and plan of care including goals, implementation of treatment, evaluation and outcomes for patients. Provides patient, family, and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections, and other therapies.

Current RN licensure with WOCN required or

Certified prepared wound, ostomy, and continence nurse (WOCN).

Valid driver’s license and car insurance.

Strong interpersonal and communication skills.
Demonstrates ability to use problem solving, critical thinking and priority setting skills.

Minimum qualifications
