Job summary

Mercy Home Health Care, an agency of Trinity Health At Home, seeks a full-time RN WOCN Consultant to support the City of Philadelphia from our South Philadelphia branch. This role provides wound care at patients’ homes with a patient-centered focus and a strong emphasis on education and outcomes.

The WOCN will coordinate treatment plans, perform comprehensive wound assessments, and educate patients, families, and staff on wound management and prevention. 40 hours weekly.

Qualifications

• Graduate of an approved nursing education program.
• Licensure as a Registered Nurse in PA.
• Three years of professional nursing experience.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide one-to-one wound care in patients' homes.
• Coordinate treatment and plan of care with goals, implementation and outcomes.
• Educate patients, families, and staff on wound management and prevention.
• Support surfaces, dressing selections, and other therapies.

Mercy Home Health Care has a need for a Full-time (1.0 FTE - 40 hours weekly) RN WOCN Consultant. This position is based from our South Philadelphia branch office and will support the City of Philadelphia.

The WOCN is responsible to provide wound care to the patients of Mercy Home Health. Coordinates treatment to provide comprehensive wound care assessments and plan of care including goals, implementation of treatment, evaluation and outcomes for patients. Provides patient, family, and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections, and other therapies.

Current RN licensure with WOCN required or

Certified prepared wound, ostomy, and continence nurse (WOCN).

Valid driver’s license and car insurance.

Strong interpersonal and communication skills.
Demonstrates ability to use problem solving, critical thinking and priority setting skills.

Minimum qualifications

• Graduate of an approved nursing education program

• One (3) years experience as a professional care nurse
