Brookhaven Health Care Facility in East Patchogue, NY is seeking a Wound Care RN to join its clinical team. This salaried, full-time role offers a Monday–Friday schedule, 8 am – 4 pm, and a competitive compensation package including a $104,000 annual salary and $50 hourly equivalent.

Qualifications

• Valid New York State RN license required.
• CPR certification required or obtained upon hire.
• Experience in skilled nursing, rehabilitation, long-term care, acute care, or wound care preferred.
• Strong clinical assessment, wound management, and critical thinking skills.
• Ability to supervise, delegate, and collaborate with interdisciplinary teams.
• Excellent communication, organizational, and documentation skills.
• Commitment to providing compassionate, resident-centered care.

Responsibilities

• Provide direct nursing care to residents per care plans and wound care protocols.
• Assess, monitor, and document wound status and treatment progress.
• Administer medications, IV therapies, and wound care as prescribed.
• Collaborate with physicians, nursing staff, therapy teams and other team members.
• Develop, implement, and update wound care plans as needed.
• Monitor for infection, skin breakdown, and changes in resident condition; report concerns.
• Supervise and support CNA and LPN staff to ensure quality care.
• Maintain accurate documentation and HIPAA-compliant records.
