Qualifications

• Active New York LPN license.
• Wound care experience in a skilled nursing or long-term care setting preferred.
• Wound care certification is a plus.
• Strong clinical assessment and documentation skills.

Responsibilities

• Perform wound assessments and provide treatment per physician orders and facility protocols.
• Monitor and document wound progress and communicate changes to the interdisciplinary team.
• Assist in developing and implementing individualized wound care plans.
• Ensure accurate and timely documentation of all treatments and assessments.
• Educate residents, families, and staff on wound prevention and treatment strategies.
• Participate in skin rounds and quality improvement initiatives.
• Maintain compliance with federal, state, and facility regulations.
• Collaborate with physicians, NPs, and nursing staff for resident outcomes.
