Job summary

Advatix, Inc. seeks a Registered Nurse specialized in wound care to join our Brooklyn team. You will perform comprehensive wound, ostomy and continence care, assess patient needs, and implement care plans under supervision of the Wound and Ostomy Manager.

Responsibilities include conducting assessments, coordinating with healthcare teams, and educating patients about prevention and self-care. A NYS RN license and wound care certification are required; CWON/CWCN/CWS/WCC credentials are preferred.

Qualifications

• Wound care certificate required.
• Active NYS RN license required.
• Certification in wound care specialties preferred (CWON/CWCN/CWS/WCC).

Responsibilities

• Conducts wound, ostomy and continence-focused assessments.
• Develops and revises plans of care.
• Documents patient responses to wound care and adjusts treatment.
• Collaborates with care teams and makes referrals as needed.
• Educates patients and families on prevention and self-care.
• Routinely assesses patients at risk for pressure injuries and initiates prevention.
• Participates in quality improvement activities.
• Ensures data confidentiality and patient rights compliance.
