Job summary

Alameda Healthcare & Wellness Center seeks a Treatment Nurse under the direction of the Director of Nursing Services. You will manage treatments prescribed by physicians for all residents and ensure proper documentation of skin conditions and care plans.

Key duties include regular skin assessments, timely notification of physicians, and collaboration with the care team to maintain standards of resident skin health. This role requires ongoing in-service participation and adherence to policy.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current licensure as a Registered Nurse or a Licensed Vocational Nurse in the state of practice.
• At least one year of experience in a skilled nursing facility.
• Ability to read, write and speak the English language.
• Possess mental and physical health.

Responsibilities

• Assess residents' skin condition by performing full body assessment on admission, readmission and returns from day, night or extended leaves.
• Initiate Skin and Wound Progress report when skin problems or pressure ulcers are identified.
• Notify attending physician promptly of any skin condition.
• Initiate all prescribed treatments and ensure orders exist for skin conditions.
• Document skin problems and pressure ulcers on the resident's Plan of Care and on care assessment forms.
• Conduct weekly skin assessments and report findings to the administrator and care team.
