Job summary

Responsibilities

• Reconcile medical records for accuracy and regulatory compliance.
• Evaluate patient responses and identify additional interventions needed to meet goals of care.
• Collaborate with the interdisciplinary care team to ensure effective care planning.
• Perform other duties as assigned.
