Responsibilities

• Perform comprehensive wound assessments and document findings.
• Provide wound care and dressing changes per physician orders.
• Assist with procedures and monitor healing progress.
• Educate patients and families on wound care, nutrition, and prevention.
• Coordinate with physicians, home health, and DME providers.
• Maintain accurate EMR documentation and support quality initiatives.
