Job summary

Alameda Health System in Alameda, CA is seeking an experienced AHD Wound Care Center Case Manager (RN) to deliver comprehensive case management for clients with complex wound conditions. You will assess, plan, implement, and evaluate care, support social needs, and collaborate with physicians to develop wound treatment plans.

The role requires an active California RN license, BLS, and wound care experience; master's preferred.

Qualifications

• Active California RN license (BLS required).
• Wound care experience preferred.
• Strong assessment, planning and coordination skills.
• Ability to work with physicians and multi-disciplinary teams.

Responsibilities

• Perform nursing assessment on admission and ongoing care planning.
• Coordinate wound care and HBO protocols with physicians.
• Develop and implement care plans with patient education and discharge planning.
• Document and communicate patient data and plan of care to the team.
• Serve as case manager for wound care patients from admission to discharge.
• Ensure infection control and equipment maintenance in clinic setting.
