Job summary

Talented Medical Solutions in Simi Valley, CA seeks an experienced RN for Wound Care Specialist duties in an in-office setting. You will assess, plan, and implement wound, ostomy, and continence care across inpatient-adjacent units as needed.

Responsibilities include rounds on patients with wounds, collaboration with physicians and nursing staff, and education for patients and families. BSN preferred; RN license required; CPR/BLS required; wound care certification preferred with plan to achieve.

Qualifications

• Bachelor's Degree in Nursing (BSN): Preferred
• Registered Nurse (RN) licensure in the state of practice: Required
• CPR or BLS certification from approved vendor per AH policy: Required
• Wound care certification: Preferred with plan to achieve in determined time period
• Basic Life Support (BLS) certification from AH policy: Required

Responsibilities

• Collects relevant data pertinent to the patient's health or situation.
• Analyzes the assessment data in determining diagnosis and care issues.
• Develops a plan that prescribes interventions to attain outcomes.
• Implements the plan, coordinates care delivery, and employs strategies to promote health and a safe environment.
• Evaluates progress toward attaining outcomes. Identifies outcomes for the patient or the patient's situation.
• Collaborates with the team of patient, family, and healthcare providers in providing patient care in a safe, healing, humane, and caring environment.
• Provides learning opportunities for patients/family members and team members.
• Directly provides health information to patients, families, and treatment team.
• Maintains appropriate supply of equipment and materials.
• Maintains complete, accurate, concise, legible documentation of patient status, interventions and responses.
• Performs other job-related duties as assigned.
