Job summary

Kindred Hospital San Antonio Central seeks a Registered Nurse to manage the facility's wound care program, provide direct patient care, and support staff nurses. You will serve as a consultative resource to affiliated agencies and lead standardization of wound care practices.

Responsibilities include developing care protocols, educating staff, evaluating wounds, and collaborating with physicians to optimize patient outcomes in a long-term acute care environment.

Qualifications

• Excellent communication and interpersonal skills for collaborating with hospital departments and patients.
• Ability to adapt, set priorities, and solve problems in a clinical setting.
• Knowledge of wound care per NPIAP/AHCPR/WOCN/CDC guidelines.

Responsibilities

• Develops and implements the facility wound care program aligned with national standards.
• Establishes standards of care, policies, and procedures for wound care.
• Acts as primary consultant to Wound Care Clinicians and supports staff with wound assessment and plans of care.
• Provides ongoing education to staff on wound care products and practices.
• Evaluates wounds at admission and throughout treatment to guide care plans.
• Collaborates with physicians to adjust wound care orders and evidence-based practices.
• Reviews medical records to ensure proper documentation of wound assessments and treatments.
• Investigates adverse wound events and works with nursing leadership on action plans.
• Participates in CMS quality data reporting and cost-effective formulary management.
