Job description

Kindred Hospital San Diego is a 70‑bed long‑term acute care facility providing in‑depth care for extended recovery. We partner with physicians and offer 24‑hour clinical care seven days a week, including an eight‑bed ICU and two negative pressure rooms, located just outside downtown San Diego, less than two miles north of the San Diego Zoo.

Job Summary

Manages the facility wound care program, including standards of care and practice related to wound, ostomy, and continence patient care needs. Provides direct patient care, supports staff nurses, and serves as a consultative service to affiliated healthcare agencies.

Essential Functions
  • Develops and implements the facility wound care program with national standardization, creating patient care protocols, documentation tools, a wound care formulary, and referral criteria.
  • Establishes standards of care, competencies, policies, and procedures that are quality, cost‑efficient, and effective for all clinical staff.
  • Acts as primary consultant to wound care clinicians, training and supporting staff so they can assess wounds, recommend protocols, and initiate care plans.
  • Provides ongoing education to staff on available products.
  • Evaluates all wounds at admission and during stay to determine treatment plans and early problem identification.
  • Provides consultation on patients with pressure injuries and on wounds that do not show measurable healing within two weeks.
  • Assists the primary nurse in developing comprehensive wound care plans in consultation with physicians.
  • Makes recommendations to physicians for changes to wound care orders, providing evidence‑based research support as needed.
  • Reviews medical records for documentation accuracy and appropriate wound‑related treatments.
  • Investigates adverse events through root cause analysis and works with nursing leadership to develop action plans.
  • Participates in clinical outcome monitoring, follow‑up, and agency performance improvement initiatives.
  • Participates in CMS quality data reporting through wound‑related LTRAX records.
  • Collaborates with Support Center Clinical Operations staff to maintain a cost‑effective wound care formulary.
  • Assists with training new hire wound care coordinators within the hospital division.
Knowledge / Skills / Abilities / Expectations
  • Effective communication and interpersonal skills for building relationships with all hospital departments and for instructing individuals and groups, including patients and families.
  • Ability to adapt to new situations, set priorities, and use problem‑solving techniques.
  • Knowledge of wound care consistent with NPIAP, AHCPR, WOCN, and CDC guidelines.
  • Knowledge of wound debridement as indicated within level and scope of practice.
  • Ability to serve as a resource to nursing staff in complex wound management.
  • Ability to lead, motivate, and develop others as a team.
  • Program management skills.
  • Fluent reading, writing, and speaking in English.
  • Good and regular attendance.

Pay Range and Benefits
