Eastern Health Group is seeking a dedicated Wound Care Nurse to assess, plan, implement, and evaluate care for residents with wounds and related conditions in a long-term care setting.

The ideal candidate will hold an active Virginia RN or LPN license, with 1–2 years of wound care experience, and prefer a Wound Care Certification. This role emphasizes clinical leadership, staff education, and adherence to regulatory standards.

Qualificações

• Active Virginia RN or LPN license is required.
• Wound Care Certification preferred.
• 1–2 years wound care experience in long-term care or skilled nursing facility.
• Strong knowledge of wound care products, protocols, and documentation.

Responsabilidades

• Conduct regular skin assessments to identify wounds or risk areas.
• Develop and implement individualized wound care plans with the interdisciplinary team.
• Perform wound treatments, dressing changes, and prescribed interventions.
• Monitor wound healing and document progress accurately.
• Educate staff on wound prevention, skin integrity, and dressing techniques.
• Collaborate with physicians, dietitians, physical therapists, and others for comprehensive care.
• Maintain compliance with state and federal wound care regulations in LTC settings.
• Participate in QAPI programs and quality improvement initiatives.
• Serve as a clinical resource and mentor for nursing staff on wound care.
