Job summary

Eastern Health Group in Virginia is seeking a dedicated Wound Care Nurse (RN or LPN) to assess, plan, and implement care for residents with wounds and skin conditions. You will work with an interdisciplinary team to ensure high-quality wound care and regulatory compliance.

The ideal candidate has 1–2 years of wound care experience, knowledge of wound care products and documentation, and strong communication and teamwork skills in a fast-paced long-term care setting.

Qualifications

• Required: Active Virginia RN or LPN license.
• Preferred: Wound Care Certification.
• Experience: Minimum 1-2 years of clinical wound care experience, preferably in a long-term care or skilled nursing facility.
• Strong knowledge of wound care products, protocols, and documentation practices.
• Excellent communication, organizational, and critical thinking skills.
• Ability to work independently and as part of a team in a fast-paced environment.

Responsibilities

• Conduct regular skin assessments on residents to identify wounds, pressure injuries, or areas at risk.
• Develop and implement individualized wound care plans in collaboration with the interdisciplinary team.
• Perform wound treatments, dressing changes, and other therapeutic interventions as prescribed.
• Monitor wound healing and document progress accurately and consistently.
• Educate staff on wound prevention strategies, skin integrity, and proper dressing techniques.
• Collaborate with physicians, dietitians, physical therapists, and other healthcare professionals to ensure comprehensive wound management.
• Maintain compliance with state and federal regulations related to wound care in long-term care settings.
• Participate in Quality Assurance and Performance Improvement (QAPI) programs.
• Serve as a clinical resource and mentor for nursing staff regarding wound care practices.
• Order and maintain wound care supplies and equipment.
• Attend and contribute to care plan meetings and other interdisciplinary team meetings.
