Job summary

Prairie Oasis in South Holland, Illinois is seeking a compassionate Wound Care Nurse to provide high-quality wound management for our residents, including assessment, debridement, and dressing changes.

You will collaborate with a multidisciplinary team, document progress, educate residents and families on best practices, and contribute to care plans that promote healing and prevent readmission.

Licensed practical nurse or registered nurse with IL license required; minimum 1 year in wound care.

Qualifications

• Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in Illinois.
• Minimum 1 year of experience as a wound care nurse.
• Strong knowledge of wound care principles, practices and best practices.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• Provide wound care management and treatment to residents, including debridement, dressing changes, and wound assessment
• Conduct comprehensive wound assessments to identify areas for improvement and develop individualized treatment plans
• Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication
• Monitor and document resident progress, updating treatment plans as necessary to ensure optimal healing and prevent complications
• Educate residents, families, and healthcare teams on wound care best practices and prevention strategies
• Participate in multidisciplinary team meetings to discuss resident care and develop comprehensive plans to achieve desired outcomes
