Job summary

Eastern Health Group in Virginia Beach is seeking a dedicated Wound Care Nurse to assess, plan, implement, and evaluate care for residents with wounds, ostomies, pressure injuries, and skin conditions. The position requires an active Virginia RN or LPN license, 1–2 years of wound care experience in long‑term care, and strong communication and teamwork skills.

• Active Virginia RN or LPN license required.
• Wound Care Certification preferred.
• 1–2 years of clinical wound care experience, preferably in long-term care or skilled nursing.
• Strong knowledge of wound care products, protocols, and documentation practices.
• Excellent communication, organizational, and critical thinking skills.
• Ability to work independently and as part of a team in a fast-paced environment.

Responsibilities

• Conduct regular skin assessments on residents to identify wounds, pressure injuries, or at-risk areas.
• Develop and implement individualized wound care plans in collaboration with the interdisciplinary team.
• Perform wound treatments, dressing changes, and other prescribed interventions.
• Monitor wound healing and document progress accurately and consistently.
• Educate staff on wound prevention strategies, skin integrity, and dressing techniques.
