Acacia Health Center at Sagewood in Phoenix, AZ seeks an RN/LPN with wound care expertise to conduct rounds, assess wounds, and coordinate treatment plans for residents. You will document care in the EMR, monitor progress, and collaborate with the wound care physician and interdisciplinary team.

Two years of nursing experience (wound care preferred) and current AZ license are required, along with CPR, English proficiency, and strong communication skills.

Qualifications

• Current Arizona RN or LPN license required.
• Minimum of two years of nursing experience, preferably in long-term care or wound care.
• Wound care certification preferred.
• Able to read, write and speak English.
• Basic mathematical skills required.
• Strong interpersonal and communication skills; leadership and teaching abilities.
• Current CPR certification.
• Computer literacy required.
• Empathy and understanding of elderly care needs.

Responsabilités

• Conduct wound rounds twice weekly with wound care physician and team.
• Perform comprehensive wound assessments (pressure injuries, skin tears, surgical wounds, vascular/neuropathic wounds).
• Ensure appropriate treatment orders and follow up with physician/providers.
• Document treatments and resident response accurately in EMR.
• Evaluate wound progress and suggest changes to physician or provider.
• Take and manage wound photos per policy and regulatory requirements.
• Implement and monitor preventative measures (pressure redistribution, offloading).
• Monitor for infection signs and notify physician and nursing leadership as needed.
