Job summary

Pinnacle Point Wellness & Rehabilitation in Missouri is seeking a Wound Care Nurse (RN or LPN/LVN) to own and advance our wound care program in a long-term care setting. You’ll lead assessments, develop care plans, administer treatments, and document progress to improve resident outcomes.

Qualifications

• Active RN or LPN/LVN license (Missouri)
• At least 1 year of wound care or treatment experience in long-term care
• Current BLS/CPR certification
• Strong clinical judgment and documentation skills

Responsibilities

• Perform comprehensive wound and skin assessments
• Develop, implement, and manage individualized wound care plans
• Administer treatments and ensure accurate, timely documentation
• Monitor healing progress and adjust care plans with physicians
• Track and report pressure ulcers and skin integrity data
• Lead prevention programs (Braden Scale, risk interventions)
• Educate and mentor nursing staff on wound care best practices
• Ensure compliance with state, federal, and facility standards
• Support Quality Assurance & Performance Improvement (QAPI) initiatives
• Maintain proper wound care supplies and resources
