Reliant Care Management Company in Missouri is seeking a licensed practical nurse (LPN) to provide compassionate patient care and support for residents in our facility.

The role includes conducting assessments, admissions duties, discharge planning, wound care, supervising CNAs, and responding to emergencies, while adhering to facility protocols and safety standards. Competitive benefits and a $10,000 sign-on bonus are offered.

Responsabilités

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
