Job summary

Pinnacle Point Wellness & Rehabilitation is seeking a Wound Care Nurse (RN or LPN/LVN) with experience in skilled nursing or long-term care to own our wound care program and drive measurable improvements for residents.

This full-time role offers a supportive clinical team, leadership opportunities in wound care, and meaningful work that directly improves resident outcomes. Missouri license, BLS/CPR, and 1+ year wound care experience required; 2+ years in SNF preferred.

Qualifications

• Active RN or LPN/LVN license (Missouri).
• At least 1 year of wound care experience in long-term care.
• 2+ years in Skilled Nursing Facility (SNF) preferred.
• Current BLS/CPR certification.

Responsibilities

• Perform comprehensive wound and skin assessments.
• Develop, implement, and manage individualized wound care plans.
• Administer treatments and ensure accurate, timely documentation.
• Monitor healing progress and adjust care plans with physicians.
• Track and report pressure ulcers and skin integrity data.
• Lead prevention programs (Braden Scale, risk interventions).
• Educate and mentor nursing staff on wound care best practices.
• Ensure compliance with state, federal, and facility standards.
• Support QAPI initiatives.
• Maintain proper wound care supplies and resources.
