Qualifications

• Ability to work independently while collaborating with nursing and interdisciplinary teams.

Responsibilities

• Assess, monitor, and document residents' wounds and skin conditions.
• Develop and implement individualized wound care treatment plans.
• Perform wound treatments and ensure their correct completion.
• Monitor wound healing and communicate changes to providers.
• Maintain accurate and timely clinical documentation.
• Identify residents at risk for skin breakdown and assist prevention strategies.
• Collaborate with nursing staff to ensure proper wound care techniques.
• Educate staff, residents, and families on wound prevention and treatment.
• Participate in care planning and quality improvement initiatives.
• Monitor treatment effectiveness and recommend adjustments as needed.
• Maintain compliance with facility policies and infection prevention standards.
