Fairmont Rehabilitation & Healthcare Center is seeking a dedicated Wound Care Nurse (RN or LPN) to join our team in Fairmont, WV. You will assess wounds, develop care plans, and collaborate with the interdisciplinary team to improve resident outcomes.

Qualifications

• Current RN or LPN license.
• Wound care experience preferred.
• Long-term care/skilled nursing experience preferred.
• Strong clinical assessment and documentation skills.
• Knowledge of wound prevention and treatment practices.
• Excellent communication and organizational skills.

Responsibilities

• Assess, monitor, and document residents' wounds and skin conditions.
• Develop and implement individualized wound care treatment plans.
• Perform wound treatments and ensure treatments are completed appropriately.
• Monitor wound healing and communicate changes to providers and the interdisciplinary team.
• Maintain accurate and timely clinical documentation.
• Identify residents at risk for skin breakdown and assist with prevention strategies.
• Collaborate with nursing staff to ensure appropriate wound care techniques are followed.
• Educate nursing staff, residents, and families on wound prevention and treatment.
• Participate in care planning and quality improvement initiatives.
• Monitor treatment effectiveness and recommend adjustments as appropriate.
