NewCourtland is seeking a dedicated Registered Nurse to join their team in Philadelphia. The role involves conducting assessments, developing wound care plans, and collaborating with an interdisciplinary team to provide optimal care. The ideal candidate will have significant experience in long-term care and a strong knowledge of wound care practices. Conduct thorough assessments of new admissions and readmissions using standardized tools such as Braden Scales.
Develop and implement wound care treatment plans for residents and evaluate the progress of treatment.
Educate and train nursing staff on wound prevention, treatment, and proper documentation.
Collaborate with interdisciplinary team members, including physicians, nursing staff, dietitians, and physical therapy, to provide optimal care.
Participate in wound care rounds, skin and nutrition committee meetings, and other quality improvement processes.
Maintain accurate records and provide reports on resident conditions, treatments, and outcomes.
Adhere to all state and federal laws, facility policies, and best practices for wound care.
Essential Skills and Qualifications:
Active Registered Nurse (RN) license in Pennsylvania.
Minimum of 3 years of direct care experience in a long-term care setting.
Knowledge and experience in wound care, including prevention and treatment.
Wound care certification preferred but not required.
Strong communication skills and the ability to collaborate effectively with the team.
Ability to maintain accurate records and follow facility protocols.
