Job Summary and Qualifications

What you will do in this role:

  • Provide case management services for inpatient and observation patients.
  • Identify patients at risk for adverse outcomes during care transitions.
  • Perform comprehensive assessments of psychosocial, medical, and discharge needs, along with resource evaluations.
  • Reassess clinical condition and develop strategies to mitigate readmission risks, including education and community resources.
  • Coordinate care plans and collaborate with healthcare teams and physicians for successful care transitions.
  • Partner with Social Services to ensure appropriate post-acute care.
  • Refer patients to social services when risk factors are identified.
  • Involve patients and families in identifying needs and setting goals.
  • Evaluate care progression using evidence-based tools and escalate issues as needed.
  • Make referrals to third-party payers and disease management programs.
  • Facilitate patient throughput, focusing on effective transitions, quality, and efficiency.
  • Document all activities and recommendations clearly for the healthcare team.
  • Align patient needs with resources to ensure safe discharges.
  • Practice adherence to the “Code of Conduct” and “Mission and Values”.

Qualifications:

  • RN License (required)
  • Associate Degree in Nursing or Nursing Diploma (required)
  • Bachelor’s Degree in Nursing (preferred)
  • 2+ years’ experience in case management or 3+ years in clinical nursing (required)
  • InterQual experience (preferred)
