Job summary

Vineyard Post Acute in Petaluma, CA is seeking a Part-Time Treatment Nurse (LVN or RN) to deliver hands-on wound care and collaborate with the care team. This position focuses on resident comfort, healing, and compassionate, professional service.

Shifts are 8:30am–5:00pm on Saturdays and Sundays, with the possibility of an additional day. Starting rates range from $41 to $60 per hour, depending on license type and experience.

Qualifications

• Valid California RN or LVN license in good standing.
• One year of wound care experience.
• SWOC or wound care certification preferred.
• Strong assessment, organization, and communication skills.

Responsibilities

• Provide primary skin and wound care under physician and DON supervision.
• Assess, document, and treat wounds, ulcers, and infections.
• Collaborate with the interdisciplinary care team on care plans.
• Maintain accurate records and treatment documentation.
• Participate in ongoing education and training on wound care.
• Monitor resident progress and adjust treatment plans.

What You'll Do:

• Provide primary skin and wound care under physician and DON supervision, focusing on resident comfort and healing.
• Assess, document, and treat various skin conditions including wounds, ulcers, and infections.
• Collaborate with the interdisciplinary care team to develop individualized care and treatment plans.
• Maintain accurate records, reports, and treatment documentation.
• Participate in ongoing education and in-service training to stay up to date on best practices in wound care.
• Monitor resident progress and adjust treatment plans as needed.
• Ensure residents are treated with dignity, compassion, and respect at all times.
• Communicate effectively with residents, families, and healthcare professionals to ensure high-quality care delivery.
• Assist with resident admissions, discharges, and transitions of care as needed.

Qualifications:

• Valid California RN or LVN license in good standing.
• One (1) year of experience in wound care required.
