Job summary

The Blossoms is seeking a Wound Care Coordinator to ensure high-quality wound care and skin integrity for residents. You will lead assessments, coordinate plans, and collaborate with physicians to optimize healing, while maintaining patient dignity and safety in daily care.

Responsibilities include direct resident care, documentation, nutritional interventions, and strict infection control in accordance with policies and procedures.

Qualifications

• Graduate of an accredited nursing program.
• Must possess a current and unencumbered RN license in the state of the facility.

Responsibilities

• Assess, plan, implement, and evaluate residents' wound and skin integrity using the nursing process.
• Document residents' skin status and medical history.
• Plan and implement wound prevention per care plan.
• Prioritize nursing actions for wound care and prevention.
• Communicate with physicians about resident wound needs.
• Initiate nutritional interventions to support wound healing.
• Provide direct resident care per policies and procedures.
• Administer treatments per physician orders.
• Maintain resident comfort, privacy, and dignity during treatments.
• Communicate effectively with residents, families, peers, and supervisors.
• Conduct rounds to ensure pericare and timely preventative ointments; address non-compliance with DON.
• Operate safely following policies and infection control procedures.

POSITION SUMMARY:

The Wound Care Coordinator reports to and receives general direction from the Director of Nursing and is responsible for delivering quality wound care and preventative measures to maintain skin integrity to assigned residents in compliance with applicable federal and state guidelines and established policies and procedures.

ESSENTIAL JOB FUNCTIONS:

• Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation and evaluation of residents wound and skin integrity
• Collecting and documentation of residents’ skin status including medical history.
• Wound care prevention and prevention through plan of care
• Sets priorities for nursing action for wound care and prevention
• Communicates with physicians regarding resident wound needs
• Initiates nutritional interventions to ensure that residents with wounds are provided nutrition to promote wound healing.
• Performs direct resident care utilizing established procedure, policies, and standards
• Administer treatments and procedures in accordance with the physician’s orders
• Maintains the comfort, privacy, and dignity of each residents while performing treatments
• Communicates and interacts effectively, courteously, and tactfully with residents, visitors, family members, peers, and supervisors
• Conducts rounds frequently to ensure that residents with incontinence are receiving pericare and preventative ointments timely. Discipline non-compliant staff accordingly with consultation from the DON
• Performs duties in a safe manner in accordance with established policies and procedures
• Performs all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves and proper disposal of soiled material

EDUCATION/EXPERIENCE:

1. Graduate of an accredited school of Nursing
2. Must possess a current and unencumbered Licensed Professional Nurse or Registered Nurse License in the state the facility resides in
