Job summary

Rivaya of Des Plaines seeks a dynamic Wound Care Coordinator to oversee wound care management and skin integrity programs for residents at an Extended Care affiliated facility in Des Plaines, IL. The role requires an active nursing license and CPR certification, plus RN or LPN experience.

You will perform nursing assessments, conduct wound care rounds, implement preventive strategies, coordinate with physicians and interdisciplinary teams, and ensure compliance with regulations and facility

Qualifications

  • Active nursing license (state as required)
  • Current CPR certification to be maintained
  • Minimum two years RN/LPN experience
  • Ability to lift and move patients/items up to 75 lbs
  • Strong verbal and written English communication
  • Ability to cope with mental and emotional stress of the position
  • Strong attention to detail for wound care management

Responsibilities

  • Utilize the nursing process for assessment, planning, implementation, and evaluation of wound and skin care
  • Conduct regular wound care rounds with timely documentation of resident skin conditions
  • Assess resident skin integrity and document relevant medical history
  • Implement preventative wound care strategies in care plans
  • Prioritize and execute nursing interventions for wound management
  • Communicate with physicians and interdisciplinary teams regarding treatment needs
  • Initiate nutritional interventions to support wound healing
  • Perform direct resident care in accordance with facility policies
  • Ensure residents with incontinence receive appropriate pericare and preventative treatments
  • Maintain a safe and clean work environment adhering to infection control protocols

We are seeking a dedicated Wound Care Coordinator to oversee wound care management and skin integrity programs for our residents. The Wound Care Nurse is responsible for providing high-quality wound care, ensuring preventative measures are in place, and maintaining compliance with federal and state regulations, as well as facility policies.

Wound Care Coordinator Requirements:
  • Active Nursing License as required by the state
  • Current CPR certification
  • Minimum two (2) years of experience as an RN or LPN
  • Ability to lift and move patients and items up to 75 lbs
  • Strong vision abilities including close, distance, peripheral, and depth perception
  • Excellent verbal and written communication skills in English
  • Ability to cope with the mental and emotional stress of the position
  • Strong attention to detail and ability to prioritize wound care needs
As a Skilled, Compassionate Wound Care Coordinator, You Will:
  • Utilize the nursing process for assessment, planning, implementation, and evaluation of wound and skin care
