ALIYA Healthcare Consulting is seeking a compassionate Wound Care Nurse to join one of our south-side facilities, delivering high-quality wound care and promoting healing.

You will assess, plan, and implement individualized treatments, educate patients and families, and collaborate with the interdisciplinary team to improve health outcomes.

Qualifications

• Requires RN license in Illinois.
• CWCN/CWS/CWOCN certification preferred.
• Minimum 2 years of wound care experience.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Assess, plan, and implement individualized wound care plans.
• Inspect wounds and document status and progress.
• Conduct wound cleansing, dressing changes, and interventions.
• Educate patients and families on wound care and prevention.
• Collaborate with interdisciplinary team for comprehensive care.
• Participate in quality improvement initiatives to improve outcomes.
• Maintain accurate, confidential patient records.

As a Wound Care Nurse with ALIYA, you will be responsible for providing high-quality wound care to our patients, promoting healing, and improving overall health outcomes.

Responsibilities

• Assess, plan, and implement individualized wound care treatment plans in accordance with established standards of care
• Conduct wound inspections and assess wound status, documenting findings and progress
• Develop and implement treatment plans, including wound cleansing, dressing changes, and other interventions as needed
• Educate patients and family members on wound care and prevention strategies
• Collaborate with interdisciplinary healthcare team to ensure comprehensive care
• Participate in quality improvement initiatives to enhance wound care outcomes
• Maintain accurate and confidential patient records

Requirements

• Minimum 2 years of experience as a Wound Care Nurse
• High School Diploma or equivalent required
• Current RN license in the state of Illinois
• Certification in wound care (CWCN, CWS, or CWOCN) preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Familiarity with electronic medical records and wound care software
