Job summary

Haralson Nursing and Rehabilitation Center is seeking a Wound Care Nurse RN/LPN for a full-time 8:00am-4:30pm shift. You will assess and manage residents with wounds and collaborate with the care team to plan and implement wound care strategies.

The ideal candidate has wound care certification and prior experience in a similar role, strong documentation skills, and a commitment to high-quality patient care in a skilled nursing setting.

Qualifications

• Wound care certification and prior experience preferred.
• RN or LPN licensure is required.
• Ability to document and communicate care plans clearly.

Responsibilities

• Conduct wound assessments and develop care plans.
• Ongoing monitoring of residents with wounds and documentation.
• Educate residents and caregivers on wound prevention and care.
• Collaborate with the care team to implement treatment plans.

Job description

Haralson Nursing and Rehab is currently seeking a Wound Care Nurse RN/LPN for Full Time 8-4:30pm. As part of the Wound Care Team, the wound care nurse participates in assessing and evaluating the clinical needs of resident with wounds, and works with the resident and the rest of the care team in planning and implementing care that addresses the needs. This work is carried out in support of the mission of Haralson Nursing and Rehabilitation Center.

Responsibilities

• Conduct comprehensive wound consultations including; detailed wound assessments and recommendations for interventions and care planning.
• Conduct wound assessments for resident and complete all associated documentation, tracking and care planning activities.
• Ongoing monitoring of all assigned resident with wounds.
• Assess resident/caregivers knowledge of wound prevention and treatment. Provide resident/ caregiver with wound prevention / treatment education as needed.

Seeking an individual with who is Wound Care Certified with Some prior experience in the role.
