Job summary

Clover Health is seeking a Wound Care Certified Nurse Practitioner to oversee care for members with complex wounds in a home-health setting in Oregon. You will develop individualized treatment plans, review wound images and data through AI-enabled platforms, and independently determine diagnoses and escalation paths.

Qualifications

Active, unrestricted NP license; ability to obtain licensure in applicable states.
Wound care certification or advanced specialty training in wound management.
3–5 years of clinical experience managing chronic/complex wounds, including pressure injuries.

Responsibilities

Manage wound care through closure with evidence-based plans; dressings, monitoring, home-health interventions.
Assess wounds using data and AI-supported tools; determine diagnosis, treatment, escalation as needed.
