Job summary

My Access Medical Clinic seeks experienced Wound Care Nurse Practitioners and Physician Assistants to lead comprehensive wound assessments, develop evidence-based treatment plans, and coordinate interdisciplinary care in long-term care settings. This role emphasizes prevention, staff and caregiver education, and maintaining high-quality resident care.

Responsibilities include diagnosing wounds, ordering diagnostics, and adjusting plans; travel to support locations as needed, with a focus on CMS

Qualifications

• MSN, DNP, MPAS, DMSc, or equivalent advanced degree.
• Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
• Must be Wound Care Certified Certification (WCC) or Certified Wound Specialist (CWS) by date of employment.
• Board Certification (AANP/ANCC/NCCPA).
• 1 year of wound care/podiatry experience in the Long-Term Care NH setting
• Excellent interpersonal communication skills required: ability to deal with the public, patients, and fellow staff in a pleasant and professional manner.
• Knowledge of professional medical practice and care to give and evaluate patient care.
• Knowledge of organizational policies, regulations, and procedures to administer patient care.
• Knowledge of medical equipment and instruments to administer patient care.
• Ability to interpret, adapt, and apply treatment protocol guidelines and procedures.
• Ability to recognize medical condition requiring consultation with the collaborating physician, and to work with the collaborating physician, in person or by phone, on these and other cases outside the scope of established protocols.

Responsibilities

• Perform comprehensive wound assessments on admission and routinely thereafter
• Diagnose and manage wound conditions (pressure injuries, ulcers, diabetic wounds, surgical wounds)
• Order and interpret diagnostics (labs, imaging, cultures)
• Develop, implement, and adjust evidence-based wound treatment plans
• Select and prescribe appropriate wound care products, dressings, and therapies
• Travel to support locations as needed
