The role emphasizes infection control, collaboration with an interdisciplinary team, and ongoing staff education to prevent skin breakdown and promote healing.

Qualifications

• Current and active LVN license in the state of employment.
• Experience in long-term care or skilled nursing preferred.
• Knowledge of wound care and documentation best practices.
• Infection control practices and regulatory standards knowledge.

Responsibilities

• Perform and document physician-ordered treatments, including wound care, dressing changes, and skin treatments.
• Assess, monitor, and document wounds and skin conditions.
• Develop and implement treatment plans with the interdisciplinary care team.
• Maintain accurate and timely documentation in the medical record.
• Monitor for signs of infection or complications and report changes.
• Ensure infection control protocols and wound care best practices are followed.
• Participate in weekly wound rounds and care team meetings.
• Educate staff on wound care procedures and prevention of skin breakdown.
• Assist with interventions to prevent pressure injuries and maintain supplies.
