Job summary

McLaren Health Care seeks a Licensed Practical Nurse to treat and manage patients with wounds and altered skin integrity. You will support, educate, and guide patients and staff within a collaborative care team.

Responsibilities include developing care plans, coordinating with medical staff on complex wounds, and educating staff on wound assessment and management while ensuring continuity of care and maintaining professional competency through ongoing education.

Qualifications

• Current state license as an LPN.
• Certification by the WOCN or eligible to become certified within 24 months of hire/transfer.

Responsibilities

• Develops, revises and implements plan of care for patients with skin integrity issues.
• Collaborates with medical staff to create treatment plans for patients with complex wounds.
• Provides education and training to nursing and medical staff on wound assessment and management.
• Evaluates patient and family learning needs and provides education to assure understanding and ability to complete self-care.
• Ensures plan of care is communicated across the care continuum.
• Maintains professional competence through continuing education.
• Performs other related duties as required and directed.
