Qualifications

• Current state license as a Licensed Practical Nurse.
• Certified or eligible for WOCN certification within 24 months of hire.

Responsibilities

• Develop, revise and implement plan of care for wounds and skin integrity issues.
• Collaborate with medical staff to create treatment plans for complex wounds.
• Provide education and training to staff on wound assessment and management.
• Assess patient learning needs and provide education for self-care.
• Ensure plan of care is communicated across the care continuum.
• Maintain professional competence through continuing education.
