Job summary

Linley Park Post Acute is seeking a Wound Nurse (LPN) to oversee and coordinate wound care services for residents in our Anderson, South Carolina facility. The role focuses on promoting healing, preventing skin breakdown, and delivering evidence-based wound care to ensure high-quality treatment.

Qualifications

• Current, unrestricted LPN license in South Carolina.
• Experience with wound care preferred.
• Skilled nursing or long-term care experience preferred.

Responsibilities

• Perform comprehensive skin and wound assessments on residents.
• Monitor, evaluate, and document wound healing progress.
• Develop and implement individualized wound treatment plans with the care team.
• Perform wound care treatments and dressing changes as ordered.
• Maintain accurate wound care documentation.
• Educate staff on wound prevention and care protocols.
• Collaborate with physicians and interdisciplinary team members.
