The role focuses on assessing skin integrity, developing wound care plans, and collaborating with physicians and an interdisciplinary team to promote healing and prevent complications.

The ideal candidate is a graduate of an accredited nursing program with an active PA LPN license, BLS/CPR certification, and experience in wound care or long-term care.

Qualifications

• Wound care knowledge and skills.
• Experience with wound assessment and treatment planning.
• Strong documentation and communication abilities.
• Ability to work within an interdisciplinary team.
• BLS/CPR certification required.
• Long-term care experience preferred.

Responsibilities

• Assess skin integrity for admissions and readmissions.
• Complete Braden Scale assessments as appropriate.
• Develop, implement, and adjust individualized wound plans.
• Evaluate responses and update care plans.
• Document assessments, treatments, and outcomes accurately.
• Collaborate with nursing leadership and physicians.
• Educate staff on wound care protocols and best practices.
• Monitor healing progress and modify treatments as needed.
