The Wound Care LPN will perform wound assessments, develop and implement care plans, document progress, and educate residents and families. Collaboration with physicians and nursing staff ensures optimal healing outcomes.

Qualifications

• Current and active New York LPN license.
• Wound care experience in a skilled nursing facility or long-term care setting preferred.
• Wound Care Certification is a plus.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational abilities.
• Passion for delivering exceptional resident-centered care.

Responsibilities

• Perform wound assessments and provide treatment per physician orders and protocols.
• Monitor and document wound progress; communicate changes to the team.

Position Summary

The Wound Care LPN is responsible for providing specialized wound care services, assisting with the prevention and treatment of pressure injuries, and ensuring optimal healing outcomes for residents in our skilled nursing facility.

Responsibilities

• Perform wound assessments and provide treatment in accordance with physician orders and facility protocols.

• Monitor and document wound progress and communicate changes to the interdisciplinary team.

• Assist in developing and implementing individualized wound care plans.

• Ensure accurate and timely documentation of all treatments and assessments.

• Educate residents, families, and staff on wound prevention and treatment strategies.

• Participate in skin rounds and quality improvement initiatives.

• Maintain compliance with all federal, state, and facility regulations and standards.

• Collaborate with physicians, nurse practitioners, and nursing staff to ensure excellent resident outcomes.
