Job summary

The Chateau at Brooklyn Rehabilitation and Nursing Center is seeking a compassionate Wound Care LPN to join our skilled nursing team in Brooklyn, NY. You will provide specialized wound care, prevent and treat pressure injuries, and help residents achieve optimal healing outcomes.

Responsibilities include wound assessments, progress documentation, care planning, and collaboration with the interdisciplinary team.

Qualifications

• Current New York LPN license required.
• Wound care experience in skilled nursing or long-term care preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational abilities.

Position Summary

The Wound Care LPN is responsible for providing specialized wound care services, assisting with the prevention and treatment of pressure injuries, and ensuring optimal healing outcomes for residents in our skilled nursing facility.

Responsibilities

• Perform wound assessments and provide treatment in accordance with physician orders and facility protocols.

• Monitor and document wound progress and communicate changes to the interdisciplinary team.

• Assist in developing and implementing individualized wound care plans.

• Ensure accurate and timely documentation of all treatments and assessments.

• Previous wound care experience in a skilled nursing facility or long-term care setting preferred.

• Wound Care Certification is a plus.

• Strong clinical assessment and documentation skills.
