The Chateau at Brooklyn Rehabilitation and Nursing Center in Brooklyn, NY is seeking a compassionate Wound Care LPN to join our skilled nursing team. You will provide specialized wound care, document progress, and contribute to individualized care plans for our residents.

Responsibilities include wound assessments, documentation, and collaboration with physicians and the care team to ensure optimal healing outcomes. A NY LPN license and prior wound care experience are preferred."

Qualifications

• Active New York LPN license.
• Experience in wound care in SNF/long-term care preferred.
• Wound care certification is a plus.
• Strong documentation and organizational skills.

Responsibilities

• Perform wound assessments and treatments per physician orders and facility protocols.
• Monitor wound progress and communicate changes to the interdisciplinary team.
• Develop and implement individualized wound care plans.
• Document all treatments and assessments accurately and timely.
• Educate residents, families, and staff on wound prevention and care strategies.
• Participate in skin rounds and quality improvement initiatives.
• Maintain compliance with federal, state, and facility regulations.
• Collaborate with physicians, NPs, and nursing staff to ensure resident outcomes.

The Chateau at Brooklyn Rehabilitation and Nursing Center is seeking a compassionate and experienced Wound Care LPN to join our exceptional nursing team. This is an exciting opportunity for a dedicated nurse who is passionate about wound management and providing outstanding care to our residents.

Position Summary

The Wound Care LPN is responsible for providing specialized wound care services, assisting with the prevention and treatment of pressure injuries, and ensuring optimal healing outcomes for residents in our skilled nursing facility.

Responsibilities

• Perform wound assessments and provide treatment in accordance with physician orders and facility protocols.

• Monitor and document wound progress and communicate changes to the interdisciplinary team.

• Assist in developing and implementing individualized wound care plans.

• Ensure accurate and timely documentation of all treatments and assessments.

• Educate residents, families, and staff on wound prevention and treatment strategies.

• Participate in skin rounds and quality improvement initiatives.

• Maintain compliance with all federal, state, and facility regulations and standards.

• Collaborate with physicians, nurse practitioners, and nursing staff to ensure excellent resident outcomes.

Qualifications

• Current and active New York Licensed Practical Nurse (LPN) license.

• Previous wound care experience in a skilled nursing facility or long-term care setting preferred.

• Wound Care Certification is a plus.

• Strong clinical assessment and documentation skills.
