Avantara Palos Heights is seeking a Wound Care Coordinator RN to join our compassionate team in Palos Heights, IL. The role focuses on supervising wound care delivery, coordinating with the treatment doctor, and supporting residents with care plans.

Qualifications

• Must have an unencumbered State of Illinois Registered Nurse (RN) license.
• CPR certification required.
• Wound care certification preferred, but a minimum of 2 years of wound care experience acceptable.

Responsibilities

• Oversee the wound care team and coordinate services under the supervision of the Director of Nursing and Assistant Director of Nursing.
• Assist residents with wound care, including lifting, turning and repositioning as part of treatment.
• Observe changes in condition, ensure a safe environment and maintain ongoing staff education.

As a Wound Care Coordinator you are instrumental in improving the lives of our residents. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara Palos Heights by providing support to the Treatment Doctor and overseeing the wound care team, assisting the patients with the delivery of wound care services under the supersvision of the Director of Nursing and Assistant Director of Nursing, with lifting, turning and repositioning of residents during the delivery of wound care, observing changes in condition, ensuring a safe environment and maintaining education. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

• At a minimum, an unencumbered State of Illinois Registered Nurse (RN);
• C.P.R. certification;
• Wound Care certification preferred, but a minimum of 2 (two) years of experience in wound care treatment is acceptable.
