The position requires a BSN, a WOC education program within 18 months, and at least 3 years of nursing experience. NJ RN license, BLS, and WOC credentials within 18 months are required.

Qualifications

Responsibilities

• Consult on complex clinical cases to improve patient care and outcomes.

About This Role

The Wound Care Coordinator is responsible for providing evidence-based nursing care for patients with gastrointestinal, genitourinary, and integumentary disorders. This role is ideal for experienced nurses who excel in collaboration and patient-centered care.

What You'll Do

• Serve as a consultant for complex clinical cases to improve patient care and outcomes.
• Assess, diagnose, and develop individualized care plans for patients with wounds, ostomy, or continence issues.
• Use evidence-based assessment techniques to identify patient problems and needs.
• Lead inter-professional teams to ensure effective, patient-centered care.
• Develop and revise processes, policies, and protocols based on evidence-based practice.
• Create protocols for the prevention of hospital-acquired pressure injuries.
• Collaborate with Nursing Education to educate staff on wound care.
• Monitor patients with pressure injuries in collaboration with Quality and Patient Safety departments.
• Evaluate new products and ensure inventory meets patient and staff needs.
• Adhere to American Nurses Association standards and modify care based on patient needs.

Requirements

Education:

• Bachelor's of Nursing required
• Completion of a WOCN Society accredited WOC education program within 18 months of employment

Experience:

• Minimum of 3 years nursing experience

Licenses & Certifications:

• NJ State Professional Registered Nurse License required
• AHA Basic Health Care Life Support HCP Certification required
• Wound, Ostomy, and Continence Certification within 18 months of entering position required
