Job summary

Harbor Health Services is seeking a Women’s Health Nurse Practitioner for the Daniel Driscoll-Neponset Health Center in Dorchester, MA. The 20-hour weekly role provides outpatient maternity-related and gynecologic care with full benefits.

The WHNP will conduct well-woman exams, contraceptive counseling, and collaborates with midwives and obstetricians to ensure timely access and continuous care for patients across Harbor practices.

Qualifications

• Completed accredited Nurse Practitioner program in Family Practice or Women’s Health.
• Board Certified NP; CNM with NP cert can substitute for WHNP.
• Current MA NP license, DEA, BLS required.

Responsibilities

• Provides outpatient maternity-related and gynecologic care for a patient panel.
• Manages care via visits, messages, referrals, and self-management support.
• Performs routine well-woman visits, history, pelvic exams, Pap smears.
• Diagnoses common reproductive disorders and treats acute reproductive issues.
• Counsels on contraception; prescribes OCs and IUDs; fertility and prenatal care.
• Oversees perimenopausal/menopausal care including hormone therapy.
• Supports midwives/OBs to ensure timely access and coordination.
• Engages population health team to integrate care across Harbor.
• Participates in meetings and quality-improvement activities.
• Documents care in EMR with progress notes and follow-ups.

We are currently seeking a Women's Health Nurse Practitioner to join our team at the Daniel Driscoll-Neponset Health Center.

Role:

The Women’s Health Nurse Practitioner provides outpatient maternity-related and gynecologic care for a panel of patients and serves as a core clinical resource for Harbor’s women’s health services. The role includes independent provision of preventive, reproductive, prenatal, postpartum, and gynecologic care; outpatient clinical support to midwives and obstetricians to promote timely access and continuity; and consultation and referral-based care for patients receiving primary care or other services across Harbor departments. Position is 20 hours weekly and offered with full benefits!

Responsibilities:

• Serves as a Women’s Health provider for a panel of patients, including outpatient maternity-related, reproductive health, preventive gynecologic, and acute gynecologic care.
• Organizes and manages the care of Women’s Health patients, including outpatient visits, patient communication by mail, email, portal, or phone, referral for specialty care, provision of or referral for patient education and self-management support, assessment of health risks, disease prevention, and evidence-based care.
• Provides routine health maintenance related to sexual and reproductive health by performing interval well woman visits. This may include a complete history and physical,developmental assessment, risk assessment, cancer screening, and health promotion education; follows up on any problems detected and makes appropriate referrals for additional services required including nutrition, social services, behavioral health, specialty care.
• Performs comprehensive women’s health care visits, including taking sexual and reproductive health histories and performing comprehensive gynecologic evaluations, including pelvic exams, clinical breast exams, and Pap smears.
• Diagnoses and treats common reproductive system disorders, including but not limited to menstrual irregularities, polycystic ovarian syndrome, endometriosis and sexually transmitted infections.
• Provides timely and expert care to patients with acute illnesses linked to reproductive health.
• Provides contraceptive counseling; prescribes oral contraceptives and long-acting reversible contraceptives; and provides fertility evaluation, prenatal support, and postpartum support.
• Inserts and removes IUDs and transdermal long-acting reversible contraception devices, including Nexplanon.
• Oversees perimenopausal and menopausal care, including hormone replacement therapy.
• Functions as a core outpatient clinical support provider for midwives and obstetricians, helping ensure timely patient access, appropriate follow-up, care coordination, and continuity when the patient’s primary obstetric provider is not onsite or otherwise available.
• Engages the population health team as necessary to support the delivery and integration of Women’s Health care services across all Harbor practices.
• Supports the organization’s efforts to support and advance the care of women across all sites
• Participates in provider staff meetings, departmental meetings, general staff meetings, trainings, care team meetings and quality improvement/practice transformation sessions.
• Complies with state continuing education laws. Enhances own clinical practice by attendance at formal and informal educational programs.
• Participates in managed care plans and assists the Center by reinforcing patient understanding of such plans; makes appropriate, cost-efficient referrals and admission decisions and refers only to participating specialists; provides guidance and monitoring of systems necessary for the function of managed care programs.
• Provides credentialing information on a timely basis.
• Updates Electronic Medical Record (EMR) appropriately using proper coding and documents care accordingly; accurately and timely documents care in the medical record with progress notes, lab follow-ups, communications with patients; maintains a current problem list, medication flow sheet, immunization record and other appropriate information.
• Achieves or exceeds budgeted visits for the year and is committed to continuously improving care and care delivery.
• During emergency operations may be required to report to work or remain at work.

Requirements:

• Completed accredited Nurse Practitioner program in Family Practice or Women’s Health, Board Certified
• Certified Nurse Midwife (CNM) with applicable NP Board certification may be substituted for WHNP
• Current Massachusetts Nurse Practitioner (NP) license, DEA Certification, MA Controlled Substance Registration and Current Basic Life Support certification required
• At least 2 years of experience working in providing maternal and gynecological clinical care as a substantial component of role
• Basic Skills in Office 365 including Word, Excel, Outlook, and Teams
• Experience with a community health center population and practice setting preferred
