Job summary

Resilience Behavioral Health in Willowbrook, Illinois, is seeking a compassionate Case Manager for our on-site team. This full-time role supports patients navigating behavioral health and substance use treatment by coordinating services, identifying needs, and connecting with community resources.

You will work with clinical and operational colleagues to promote continuity of care, document activities, and maintain confidentiality.

Qualifications

• High school diploma or GED required.
• Minimum of 1-2 years of relevant experience in case management, behavioral health, human services, or a related support services setting.
• Ability to maintain accurate documentation and protect confidential patient information.

Responsibilities

• Coordinate case management services to support patients receiving behavioral health and substance use disorder treatment.
• Assist patients in identifying needs, accessing appropriate community resources, and connecting with supportive services.
• Collaborate with clinical providers and interdisciplinary team members to promote continuity of care and support individualized treatment goals.
• Help coordinate referrals, follow-up services, and transitions between levels of care as appropriate.
• Maintain accurate, timely documentation of case management activities, referrals, and patient interactions in accordance with organizational standards.
• Communicate professionally and compassionately with patients, families, and treatment team members while maintaining appropriate confidentiality and professional boundaries.
• Identify barriers to accessing care and work collaboratively with the treatment team to support practical, patient-centered solutions.
