Job summary

Valle del Sol is seeking a Nutrition Education Specialist to assist patients with access to CHNP and WIC services, including counseling, education, and care planning. You will assess nutritional needs, determine eligibility, and support referrals to other providers.

The role requires strong communication, familiarity with EHRs, and a commitment to culturally sensitive care within a community health center setting.

Qualifications

• 18 years of age or older.
• Diet Tech Registered (DTR) or Bachelor’s Degree and no experience; or
• Bachelor’s Degree with WIC experience

Responsibilities

• Assists WIC patients with information and services for CHNP, including FMNP and other programs.
• Interviews patients, documents income status and current living conditions, and determines eligibility for WIC services.
• Evaluates patients’ nutrition/breastfeeding and health-related needs, discusses and documents eating habits, health history and current medical condition, and develops personal goals and a nutrition/breastfeeding care plan.
• Counsels and educates patients on proper nutrition/breastfeeding practices, provides educational material and refers patients to CHNP and Valle del Sol programs.
• Measures height/weight and hemoglobin testing.
• Maintains patient records and reports data on program participation.
• Assists with community outreach programs and special projects.
• Maintains confidentiality of work-related issues, patients’ records, and restricted information.
