Job summary

Qualifications

• Must have strong verbal and written communication skills.
• Proficient in Microsoft Office 365 and familiar with Electronic Health Records systems.
• Must be able to provide culturally sensitive services.

Responsibilities

• Assist patients with information and services for CHNP including WIC, FMNP, and related programs.
• Interview patients and determine eligibility for WIC services.
• Evaluate and document patients’ nutrition and health needs and develop care plans.
