Qualifications

• High School Diploma or GED required; college degree preferred.
• Bilingual in English and Spanish is required; strong communication skills.
• Minimum 1 year experience; 2-4 years preferred.

Responsibilities

• Meet with clients daily to discuss options and provide resources.
• Offer emotional support and assess client needs.
• Educate clients on available resources and assist in decisions.
• Maintain client records and follow up on interventions.
• Coordinate with healthcare professionals to ensure quality care.
• Maintain client confidentiality per policies.
• Provide weekly data/reports to supervisor.
