Job summary

Colonial Healthcare, part of the Optum family, is seeking an Annual Wellness Visit Coordinator to prepare patient charts and ensure medical information is up to date for wellness visits. You will support the provider team, educate patients, and coordinate care across clinics in Sumter, SC.

Required CCMA or nursing license, HS diploma, EMR knowledge, and HIPAA compliance; competitive pay and comprehensive benefits available.

Qualifications

• HS degree or equivalent required.
• CCMA certification or RN/LPN license or higher.
• Knowledge of EMR and HIPAA requirements.
• Familiarity with medical terminology.

Responsibilities

• Review patient charts and paperwork prior to wellness visits.
• Educate patients about procedures, vaccinations, and testing.
• Coordinate care with providers, radiology, lab, and pharmacy.
• Close care gaps and complete administrative tasks in EMR.
