Responsibilities

• Provide education to clients about managing mental illness and substance use disorders.
• Arrange transportation as needed for client mobility.
• Connect clients with community resources and develop natural supports.
• Help clients improve interpersonal and living skills.
• Complete required documentation timely and accurately.
• Ensure adherence to program policies and standards.
