Job summary

4Ever Young Gilbert is seeking an experienced Nurse Practitioner or Physician Assistant to lead our wellness program in Gilbert, AZ. You will guide patients from initial consultation to personalized treatment plans, which may include bio-identical hormone therapy, peptide therapy, medical weight loss, IV vitamin therapy, and nutraceuticals.

The role is 30–40 hours per week with no late nights or weekends, offering competitive pay and opportunities for advancement within a boutique wellness

Qualifications

• Active, unrestricted Arizona license as a NP/PA or higher.
• Active DEA registration with prescribing authority.
• 2+ years in functional medicine, hormone replacement therapy, or medical weight loss.
• Experience managing BHRT, TRT, peptides, and GLP-1 weight loss patients.
• Excellent interpersonal and consultative communication skills.
• Comfortable with EMR/practice management software LifeFile.

Responsibilities

• Conduct in-depth consultations with new patients and review health histories and goals.
• Perform physical exams and order comprehensive blood panels.
• Design individualized treatment plans and manage medications including controlled substances.
• Perform pellet insertions for hormone therapy patients.
• Educate patients and build long-term relationships with clear communication.
• Collaborate with aestheticians and front-desk staff for integrated patient journeys.
• Serve as clinical resource and train staff on wellness protocols.
• Maintain accurate charting and HIPAA-compliant documentation.

We are seeking an experienced Nurse Practitioner or Physician Assistant to lead our wellness program and help patients feel their best. You will guide patients from their very first consultation through a personalized treatment plan that may include bio-identical hormone replacement therapy (HRT), peptide therapy, medical weight loss, IV vitamin therapy, and nutraceutical supplementation. This position is 30 to 40 hours per week, with no late nights or weekends required.

Key Responsibilities

• Conduct in-depth consultations with new patients, reviewing health histories, symptoms, and goals such as energy, weight management, sleep, libido, and overall vitality.
• Perform physical exams and order comprehensive blood panels.
• Complete thorough lab reviews and health profiles, and design individualized treatment plans.
• Prescribe and manage medications, including controlled substances such as testosterone, in accordance with DEA and Arizona regulations.
• Perform pellet insertions for hormone therapy patients.
• Build trusting, long-term relationships with patients through education, clear communication, and consistent follow-through.
• Collaborate with our aestheticians and front-desk team to deliver an integrated patient journey across wellness and aesthetics.
• Serve as the clinical resource for wellness protocols, helping train staff and your medical assistant keeping them current on best practices.
• Maintain accurate, timely charting and medical documentation in accordance with HIPAA and state compliance standards.

Qualifications

• Licensure: Active, unrestricted Arizona license as a Nurse Practitioner (NP), Physician Assistant (PA), or higher (NMD/DO).
• DEA Registration: Active DEA registration, including prescribing authority for controlled substances.
• Experience: 2+ years of hands-on experience in functional medicine, hormone replacement therapy, and/or medical weight loss.
• Experience managing BHRT, TRT, peptides, and GLP-1 medical weight loss patients (semaglutide, tirzepatide).
• Excellent interpersonal and consultative communication skills, with a genuine passion for patient education.
• Comfortable with EMR/practice management software, LifeFile and detailed clinical documentation.

Preferred Skills (Big Pluses)

• Aesthetic injecting experience with neurotoxins, dermal fillers, and biostimulators.
• Functional medicine certification through IFM, A4M, or a similar program.

Compensation & Benefits

• Hourly base pay of $60 to $80 per hour, plus bonus opportunities.
